Age Friendly Community Planning Outreach 


An age-friendly community (AFC) is a community where policies, services and physical spaces are designed to enable people of all ages to live in a secure and accessible physical and social environment. AFCs contribute to good health and allow people to continue to participate fully in society throughout their lifetime. The Ministry of Seniors Affairs (MSA) is helping to implement AFCs across the province through its Action Plan for Seniors and the Finding the Right Fit AFC Planning Guide and the AFC Planning Grant Program. 

Age-Friendly Simcoe County


From the World Health Organization (WHO) to our municipal government, there is a call for an age-friendly society.  The County of Simcoe recognizes the Age-Friendly Community (AFC) concept as an initiative that falls within their mandate and, in support of our aging population, has chosen to develop a framework for the communities that reside within its geographical area.  Developing a framework will help our community stakeholders identify, plan and address needs and priorities related to older adults in our area.  

Home For Life – Enabling Seniors to Stay at Home


Home for Life recruits and trains volunteers to provide some support services to people who need help to remain confident in their home as long as possible. Some of the services provided by volunteers are friendly visiting, dog walking, shopping assistance, meal preparation, lawn care, and driving to appointments.  Other Home for Life volunteers provide computer training to new users for things like: how to use a computer, surf the Internet, use email, and some free online services such as Skype which keep people connected to family and friends.

Canadian Mental Health Association  The Canadian Mental Health Association (CMHA), founded in 1918, is one of the oldest voluntary organizations in Canada. Each year, we provide services and supports to more than 1.3 million Canadians through the combined efforts of more than 11,000 volunteers and staff across Canada in over 330 communities.

Retirement Homes Regulatory Authority (RHRA) Reporting Website  By law, all retirement homes in Ontario are required to have a procedure for responding to complaints about the operation of the home. In addition, all staff members working in the home are required to be trained on the home’s complaints procedure. You can find the home’s complaints procedure in the information package provided to you or you can ask the home for a copy.

An age-friendly community recognizes that information needs to be easily accessible and shared using different methods to make sure that information on important community events and services is delivered in formats appropriate for older adults with varying abilities and resources.

A Friendly Voice – A Friendly Voice is a free, confidential “warm line” for older Ontarians, 55+ who just want to chat with a friendly person who cares, without expectations or judgment. Whether you’re struggling with loneliness or simply looking to stay socially healthy and add some connection to your routine, we offer an empathetic ear and good conversation.​

About Your Money Seniors – Your Money Seniors is a set of three, free, one-hour, non-commercial seminars for Canadian seniors. Seminars are presented by local bankers who volunteer their time and expertise to deliver the seminar to seniors’ groups.

Aging with Confidence – Ontario’s Action Plan for Seniors  – Read our plan to support seniors at all stages of life, so that they can remain independent, healthy and active, safe and socially connected.   

Accessible Parking Application – Service Ontario – How to get an accessible parking permit. Accessible parking permits are issued to a person or business and not a vehicle.  Permit holders must have been in the vehicle and the permit displayed on the dashboard in order to park in an accessible parking space.  

Accreditation Canada – Accreditation Canada is a not-for-profit organization that is dedicated to working with patients, policy makers and the public to improve the quality of health and social services for all.  Accreditation Canada works to bring the best of health care from around the world home (and vice versa) and works closely with health and social services organizations in Canada and abroad to develop a sustainable culture of improvement that betters safety and efficiency, working to save and improve lives. From the standards used to the frontline assessments conducted and everything in between, patients and families are full partners in what Accreditation Canada does.

Age-Friendly Communities Planning Outreach Initiative – An age-friendly community (AFC) is a community where policies, services and physical spaces are designed to enable people of all ages to live in a secure and accessible physical and social environment. Age-Friendly Rural and Remote Communities: A Guide – In September 2006, the Federal/Provincial/Territorial (F/P/T) Ministers. Responsible for Seniors endorsed the Age-Friendly Rural/Remote Communities Initiative (AFRRCI). The Initiative has two main objectives: 1. to increase awareness of what seniors need to maintain active, healthy and productive lives within their communities by identifying indicators of agefriendly rural or remote communities; and 2. to produce a practical guide that rural and remote communities across Canada can use to identify common barriers, and to foster dialogue and action that supports the development of age-friendly communities.

Tablets and smartphones are particularly well supplied with apps that help those affected by Alzheimer’s disease; both the patient and caregiver.

Canadian Anti-Fraud Centre – The Canadian Anti-Fraud Centre (CAFC) is the central agency in Canada that collects information and criminal intelligence on such matters as mass marketing fraud (e.g., telemarketing), advance fee fraud (e.g., West African letters), Internet fraud and identification theft complaints.

The Change Foundation is an independent health policy think-tank that works to inform positive change in Ontario’s health care system with a firm commitment to engaging the voices of patientsfamily caregivers and health and community care providers.

ConnectUs​ – A new program called “ConnectUs” is a digital engagement program designed for seniors. This program is a partnership with Georgian College and the Central Ontario Health Teams and focussed towards  specialized populations engaging older adults in online/digital learning and improved digital literacy.  This poster is intended for agencies, like The Alzheimer’s Society, to help make the referrals to their clients.

First Link® – Whether you are living with Alzheimer’s disease, another form of dementia, or supporting someone who is, our First Link® program connects you and your family to your local Alzheimer Society programs and other community services. First Link is about helping you and your family make informed choices so you can live better with dementia at every stage of the disease. Connecting with First Link means you can:  Receive one-on-one or group support; Be referred to local healthcare providers and community services; Meet other people in similar circumstances and exchange experiences; get the help you need to plan your future.

(GAATES) Global Alliance on Accessible Technologies and Environments – is the leading international organization dedicated to the promotion of accessibility of the built and virtual environments and to promoting the Guiding Principles of the United Nations Convention on the Rights of Persons with Disabilities (CRPD).

Global Age-Friendly Cities – A Guide by (WHO) World Health Organization –  An age-friendly city encourages active ageing by optimizing opportunities for health, participation and security in order to enhance quality of life as people age.  In practical terms, an age-friendly city adapts its structures and services to be accessible to and inclusive of older people with varying needs and capacities.

Helping Hands – Community Support Services for Seniors and Adults with Physical Disabilities – Helping Hands maintains a unique service model by providing a wide range of home care and community support services for seniors and adults with a physical disability. These services are subsidized by government funding and utilize the generosity of volunteers. By relying on volunteers, Helping Hands is able to keep its costs as affordable as possible.

Health Quality Ontario – Palliative Care Report – At Health Quality Ontario, palliative care is one of three areas of emerging focus in our new strategic plan (along with mental health and addictions care and primary care). This report bolsters our understanding of the palliative care services that Ontarians receive at the end of their life – such as those in hospital, in the home (including home visits from doctors) and unplanned visits to the emergency department.

Home and Vehicle Modification Program (March of Dimes Canada) – March of Dimes Canada’s Home & Vehicle Modification Program provides funding for basic home and/or vehicle modifications. The program is intended to assist permanent Ontario residents with a substantial impairment expected to last one year or more, with their disability management.

Physical Activity Toolkit for Older Adults – ParticipACTION Newfoundland Labrador – For the first time ever, older adults make up a greater share of Canada’s population than children, with Atlantic Provinces having the highest share of older adults in the country. By 2036 in Newfoundland and Labrador, 31 per cent of our population will be over 65 years of age.  We know that one way older adults can maintain and enhance their well-being and independence is through a lifestyle that embraces physical activity. Volunteer Connection – Volunteer Connection is a program that Community Connection runs to promote and support volunteerism in an effort to connect all people to meaningful volunteer opportunities.  Services include a referral and matching service for volunteer placement within the community for all ages, education and support for volunteers and community service agencies and organizations through topic and issue related workshops, and community outreach.

10 Early Signs and Symptoms of Alzheimer’s – Memory loss that disrupts daily life may be a symptom of Alzheimer’s or other dementia. Alzheimer’s is a brain disease that causes a slow decline in memory, thinking and reasoning skills. Physical Activity Toolkit for Older Adults – ParticipACTION Newfoundland Labrador  For the first time ever, older adults make up a greater share of Canada’s population than children, with Atlantic Provinces having the highest share of older adults in the country. Prevention of Senior Abuse Network, SimcoeCount (PSAN-SC)  – In 1997 the Long-Term Care Committee of the Simcoe County District Health Council identified the need for a community response protocol to address abuse issues. The Abuse Protocol Steering Committee created a document outlining an intervention model in Simcoe County. Shortly after this committee determined it should focus on seniors and The Simcoe County Elder Abuse Committee was formed.

Community Connection – A discussion on Social Isolation and Social Connection in Simcoe County – Community Connection is a non-profit information and referral service accredited by the Alliance of Information and Referral Systems (AIRS). Our mission is to create and improve access to services for people.  The core areas of work are, providing call centre services for 211, a national 24/7 helpline that connects people to essential services; maintaining a database of community resources and supporting online directories for public access; and producing reports on community needs/trends, and service gaps to support community planning.  Another important area of our work is in our community where we are involved in numerous partnerships, shared service initiatives, and collective impact projects that are collaborative approaches to improve and support access to services for people. Several years ago, Community Connection established a volunteer program with the goal to connect volunteers to organizations and increase awareness of the impact of volunteering in our community.  A recent survey completed by more than 800 people over the age of 55 living in Simcoe County has provided insight to new perspectives on social engagement. Community Connection – A discussion on Social Isolation and Social Connection in Simcoe County – Community Connection is a non-profit information and referral service accredited by the Alliance of Information and Referral Systems (AIRS). Our mission is to create and improve access to services for people.  The core areas of work are, providing call centre services for 211, a national 24/7 helpline that connects people to essential services; maintaining a database of community resources and supporting online directories for public access; and producing reports on community needs/trends, and service gaps to support community planning.  Another important area of our work is in our community where we are involved in numerous partnerships, shared service initiatives, and collective impact projects that are collaborative approaches to improve and support access to services for people. Several years ago, Community Connection established a volunteer program with the goal to connect volunteers to organizations and increase awareness of the impact of volunteering in our community.  A recent survey completed by more than 800 people over the age of 55 living in Simcoe County has provided insight to new perspectives on social engagement. The project team made up of a cross-sector of community leaders and service providers aimed to “Reducing Senior Social Isolation” in Simcoe County, surveyed seniors during the month of September 2021 for feedback on their level of social activity, their interests and how best to become or stay involved in th​​eir communities in meaningful ways.  Click on the link the following link to view the survey report:  July 2022 Report

ConnectUs​ – A new program called “ConnectUs” is a digital engagement program designed for seniors. This program is a partnership with Georgian College and the Central Ontario Health Teams and focussed towards  specialized populations engaging older adults in online/digital learning and improved digital literacy.  This poster is intended for agencies, like The Alzheimer’s Society, to help make the referrals to their clients. ​​

Dancing Makes You Smarter – Use It or Lose It: Dancing Makes You Smarter, Longer.  Growing evidence reveals that stimulating one’s mind by dancing can ward off Alzheimer’s disease and other dementias, much as physical exercise can keep the body fit.  Dancing also increases cognitive acuity at all ages.

ElderDog Canada – ElderDog Canada Inc. is a national, registered Charitable Organization dedicated to ageing people, ageing dogs, and the important connection they enjoy. Our purposes are to:   Assist and support older adults in the care and well-being of their canine companions; Promote the welfare of older dogs whose lives have been disrupted due to illness, relocation or death of a human companion; Educate about the human-animal bond in general and the significant role of companion animals in the health and well-being of seniors; Support and conduct research into the human-animal bond with a focus on seniors and canine companionship.

GrandPals​ – ​www.grandpals.ca –  An Age-Friendly program that incorporates respect, social inclusion, and social participation among older adults and children. GrandPals Mission – Creating, facilitating, and promoting curriculum integrated, intergenerational experiences (focused on shared learning, storytelling & friendship). ​  GrandPals was formed to build a world of deeper intergenerational learning, storytelling and friendship by engaging older adults with students at an elementary school level.  ​

Pen Pal Program – Seniors Programs and Services (District of Muskoka):  

The purpose of the Pen Pal Program is to provide older adults living in rural areas with the opportunity to make new friends via written communication and experience the joy of receiving a regular piece of mail. Through a matching process the Pen Pal volunteers will be matched up with clients who have applied to the program.

Red Hat Society – Experience the Fun of RHS – The Red Hat Society is a playgroup for women created to connect like-minded women, make new friends and enrich lives through the power of fun and friendship.

Volunteer with the SMIFS Simcoe Muskoka Cyber Seniors Program – https://www.simcoe.ca/LongTermCare/Pages/SMIFS-Simcoe-Muskoka-Cyber-Seniors-Program.aspx – The goal of this program is to increase family and social connectedness in seniors to reduce the risk of isolation.  The 14 week program introduces the senior to the basics of an iPad, then walks them through using email, texting, FaceTime or video calls, in an effort to make communication with family more accessible.  Facebook and YouTube will also be explored as will any other interest of the senior.
