ABOUT THE HART (Homelessness and Addiction Recovery Treatment) HUB MODEL
The vision is to effectively support the treatment and recovery of individuals with complex service needs in local community-based settings. Designed to support highly marginalized populations, including those at risk of, or who are experiencing homelessness, through access to a suite of integrated, locally tailored health and human services including, but not limited to, mental health and addictions services, primary care and supportive housing, in conjunction with case management services, to navigate additional social and employment services.
WHO WE ARE
We know that, like in many communities, across our region there is a need for integrated community-based services to address the homelessness health crisis.
We believe that these considerable investments in mental health and addictions through the HART Hub Model will make a significant and lasting impact for unhoused individuals who are working to overcome addictions.
This is not something that one organization can do alone.
By working with our partners, we are confident that those using the HART Hub will have the support they need to get a healthy fresh start on life.

FAQ
Simcoe County’s HART Hub model, known as The HART of Simcoe County, aims to integrate key services related to addictions and mental health, including both recovery and treatment, supportive housing, primary health care, and other social and human services. The vision is to enhance and build upon existing services while clearly defining referral pathways to ensure timely access for those in need. Rather than centralizing all services in one physical location, the integrated service model focuses on creating access points, pathways, and connections to support a person-centered, accessible, and low-barrier approach.
The design of the HART of Simcoe County also aims to create opportunity for community-based social and human services and primary health care to co-locate where it makes sense to do so, with the goal of increasing and simplifying access to critical services. As the project is currently in the planning and implementation phase, working closely with our partners and planning to engage further with community members, we are still exploring the best locations and how these services will work together.
The HART of Simcoe County model is designed through a multi-phase approach to support the creation and enhancement to key and primary services, including supportive housing, mental health and addiction recovery and treatment services, access to primary care, amongst other critical services. We are expecting further information from the Ministry of Health related to implementation and funding; however, we do anticipate that initial phases of services will be available in Spring 2025. Further updates will be provided when additional information is available.

- Clinically based addiction and mental health services
- Crisis management and support
- Primary healthcare and wellness support

- Support in accessing social assistance programs and services
